Informed Consent: I understand that Reiki is a natural healing system that is believed to promote the body’s natural healing capacity and balance in body and mind. I understand that Reiki practitioners do not diagnose conditions, they do not prescribe medicines or perform medical treatment, nor interfere with the treatment of a licensed medical professional. I understand that I am advised to seek advice from a licenced physician or licensed health care professional for any physical or psychological ailment or medical concerns I may have without delay and will not make any adjustment to any medical professionals advice without consulting a medical professional. As Reiki treatments are understood to promote a greater state of balance in mind and body, the possibility exists that if you have been prescribed a medication regime for any imbalance, this may require more regular monitor-ing, re-evaluation and management over time by the prescriber. Reiki can however complement any medical or psychological care I may be receiving. I acknowledge that long term imbalances in the body commonly re-quire multiple sessions in order to facilitate the level of relaxation needed by the body to heal. I understand that it is not possible to guarantee the outcome of any course of treatment and that I may become aware of some beneficial affects over an extended period of time after the Reiki treatments.

Data Privacy: I understand that all information will be treated in the strictest confidence, save for circumstances where the practitioner has a legal responsibility to report. I confirm that I am over 16 years of age and I testify that all the details I provide are correct to the best of my knowledge, I have not withheld any relevant information and that if any personal information changes, I accept that I must inform the practitioner accordingly. The full privacy policy is available from the practitioner on request at this point. I understand that my data will only be used for direct communications in connection with the delivery of services I request.

Cooling off period: I understand that I have 14 days after payment to change my mind about receiving treatments and can ask for a refund during this time only if the course of treatment sessions have not yet started. Once the first treatment has started, I understand that the number of sessions I have purchased is considered a course of therapy and I can no longer apply for a refund.

Consent to proceed with the Reiki treatments: I confirm that the practitioner has fully explained the treatment and the procedures involved. I have had opportunity to ask any questions regarding the treatment, all questions have been answered to my satisfaction and wish to proceed with Reiki treatments.
